A report to Police Regional Office 3 director Brig. Gen. Ponce Rogelio Peñones Jr. by the Pampanga police showed the tragedy occurred in front of the Our Lady of Fatima University in the City of San Fernando.

The driver was later turned over to police on the same day for proper investigation and documentation.
Probers are reviewing the closed-circuit television (CCTV) footages from nearby establishments to determine the exact circumstances surrounding the incident.
Charges of reckless imprudence resulting in homicide are being prepared for filing in court against the jeepney driver.
